Note: All involved personnel shall be made aware of their function and
responsibilities included in the procedure.
9 HARNESSES & SAFETY EQUIPMENT
Harness must be worn at all times during man-riding operations, Harness can be
worn in different configurations and the appropriate securing equipment must be
available i.e. safety reels, lanyards to allow for work restraint, fall arrest, and work
positioning.
9.1 HARD HATS
Hard hats should be fitted with a chin strap to ensure that they are NOT capable of
falling from an elevated work site.
10 RESCUE PLANS
It is difficult for one procedure to specify an appropriate rescue plan for an explicit
site, however, each BP site where people are exposed to work at height or man
riding operations must have a rescue plan and this must form an integral part of the
emergency control procedures.
Most plans will deal with the rescue of a person left suspended in either fall arrest,
work positioning or rope access equipment. For the latter, the specialist contractor
will already have a plan, equipment and personnel as part of their standard
procedures. In the case of fall arrest or work positioning equipment, a separate plan
should be developed as follows:

Note: Most service providers will supply rescue equipment and competent personnel
to perform this work. The worksite may deem this adequate however it is
recommended that on sites such as platforms, personnel in the fire team/rescue
team are provided with the appropriate equipment to perform this task.
Consider the level of medical treatment that may be required (refer to the following
paragraph)
Suspension trauma is a medically recognised condition. Everybody who is
suspended in a safety harness runs the risk of shock and unconsciousness due to
blood flow insufficiency. Unconsciousness can become life threatening after only a
few minutes.
Shock, caused by a lack of blood flow, is due to the blood accumulating in the lower
parts of the body as a result of the muscles relaxing and the „muscle pump‟ effect
stopping. The need for rapid and effective rescue is particularly important when using
personal protective systems where a delay may have severe consequences, i.e.
when someone is left hanging motionless in a harness after a fall. The time before
loss of consciousness can vary from about 6 minutes to 2 hours, depending on
incident severity.

11 MAN BASKET TRANSFER OPERATING GUIDELINES
The use of a crane to hoist personnel from one structure or vessel to another is a
potentially hazardous operation if not carried out correctly.
The use of a man basket will only be sanctioned after a Formal Risk Assessment and
may only proceed if no other means of access is available or practicable.
Note: Any equipment not specifically designated for lifting persons shall not be
used in this manner.
11.1 OPERATING REQUIREMENTS
The duties of people in supervising or actually carrying out the personnel basket
transfer shall be clearly defined. People usually covered are the Installation Manager,
the crane operator, the Master of the Vessel, and other people nominated by the OIM
or the ship's Master to undertake specific duties. The procedure setting out those
involved in the transfer operation should cover the following:

 Be aware of the suitability of the crane for personnel transfer
 Check the wind speed limitations on crane operations
 Establish and maintain communication with the Master of the Vessel
 Brief the Master of the Vessel
 Be satisfied with the competence and experience of the crane operator
 Ensure appropriate emergency precautions are in place, e.g. notify the
standby vessel before the transfer
 Be satisfied with the inspection and testing of the personnel basket
The Master of the Vessel should confirm to the OIM that:
 The transfer has been accepted and the procedures understood
 The vessel has a satisfactory station keeping capability
 The deck crew have been fully briefed
 The people to be transferred have been adequately briefed and are fit to be
transferred
The crane operator should establish that:
 The crane is fully operational
 The wind speed is satisfactory for safe operation
 The requirements and procedures have been understood
 The banks-man has an unobstructed view and the transfer areas are clear
 Adequate communications have been established with the vessel Master and

In addition, an operator / mechanic, capable of operating the crane, shall be available
in case of an emergency.

 The personnel basket is correctly used
 The individuals are fit to transfer and have understood the procedures
 Proper communications have been established
 They each have a full view of the transfer area
Individuals to be transferred should:
 Ensure that the transfer procedure is understood
 Confirm they agree to the transfer
 Be able to use correctly the safety equipment provided
Man riding operations shall only take place in daylight and in good visibility:
 The basket shall not be used if wind speed is in excess of 7 metres/second,
(14 knots) or the limitations determined by Risk Assessment, if lower.
 Man baskets should not be used in the following conditions:

11.2 SUITABILITY OF VESSELS FOR TRANSFER OPERATIONS
The type of vessel considered suitable to carry out a transfer should be determined
by its ability to maintain station alongside the installation and should have sufficient
clear deck space to safely receive the basket.
11.3 WEATHER CONDITIONS
Weather conditions are very important factors which should be taken into account,
and include visibility, wind and sea state. Duty holders should specify the maximum
wind speed and sea state beyond which basket transfer is not permissible and the
wind speed limitations for the crane operations.
11.4 COMMUNICATIONS
Both radio and visual communication should be established and maintained between
the people actually conducting the operation.
11.5 SAFETY EQUIPMENT AND RESCUE PROCEDURES
People being transferred shall wear self inflating life jackets, suitable clothing and
immersion suits. All life jackets should be equipped with suitable means of
illumination during night transfers. Appropriate rescue and recovery arrangements
must be in place (e.g. standby vessel equipped with a fast rescue craft ready to
launch).
11.6 TRAINING
Personnel will be transferred by basket in greater safety and with less apprehension
if they have received training in the techniques involved. The type of training should
be included in installation drills. Inexperienced people or those not trained in the use
of personnel baskets should always be accompanied. At least one person in each
transfer must have previously ridden in a man basket.
Note: Transfer by basket is voluntary.
11.7 PRE-TRANSFER OPERATIONS
Prior to transfer operations:
 A toolbox talk will be held for all relevant personnel
 The basket shall be thoroughly inspected by the person supervising the
transfer
 It must be ensured that landing and lifting areas are clear of all obstructions
and that there is sufficient space to allow secure and safe operation of the
basket

 The standby vessel shall be informed of the impending transfer

life jackets of an approved type.
 Effective emergency arrangements must be in place throughout the transfer,
the rescue and recovery arrangements must be capable of recovering people
from the sea within specified performance standards. Where transfer
operations over water are carried out in harbours, passengers should be
provided with life jackets as a minimum requirement
11.8 TRANSFER OPERATIONS
During transfer operations:
 The crane driver shall concentrate completely on the transfer operations
 The banks man shall have visual contact with the crane operator and the
basket at all times
 Direct radio contact shall be maintained at al times between the basket, crane
operator, and banks man
 All instructions shall be given in metres where radios are employed for
communication
 The basket shall only be lifted to a height sufficient to clear any obstacles
prior to being lowered.
11.9 POST-TRANSFER OPERATIONS
The person supervising the transfer operation is responsible for guiding transfer
personnel to registration and to the person who will provide their induction.

12.1 APPENDIX A: PRE START CHECKLIST
Due to Specific requirements for man-ridding operations, the suitability of cranes and
winches must be assessed for each and every operation. It is mandatory for this
check sheet to be filled in and accompanied with the work permit and TRA before
any man-ridding operations commence.
If the answer to any of the questions is „No‟ then permission must be granted by the
Site Manager / Site Controller / Offshore Installation Manager.

INTRODUCTION
1.1
Purpose
The purpose of this Permit to Work (PTW) procedure is to ensure that the controls necessary
are available to provide safe performance for work against a specific range of potentially
hazardous tasks. These activities are explained in Section 6. This procedure shall be used in
alignment with the suite of BP AzSPU SSOW procedures as identified in section 1.9.
PTW is the generic term that refers to documents for controlling work. These documents are
designed to cover different work activities and will be addressed within this procedure.
1.2
Scope
The contents of this Safe System of Work are applicable to all BP owned and managed
sites/installations in Azerbaijan and Georgia. Contractors working on BP owned or managed
sites/installations are also responsible for alignment with this SSOW.
This document does not replace the procedures prepared and adopted by specialist contractors.
Neither does it supersede any national and local regulatory requirements.
This SSOW contributes to compliance with the “HSE expectations” contained in “getting HSE
right”, the „Golden Rules of Safety‟ and the Control of Work (CoW) standard that the Hazards
associated with BP activities are identified and that the Risks are assessed and managed.
All guidelines contained shall be regarded as the minimum requirements for BP owned or
managed sites in Azerbaijan and Georgia.
The scope covers defined activities of BP staff and Contractors at all BP AzSPU sites and
installations.
1.3
Legislation & Standards
The aim of this Safe System of Work is to achieve no accidents, no harm to people and no
damage to the environment. To achieve this aim, this SSOW complies with National Legislation,
the terms of the Production Sharing Agreement (PSA) and mandatory BP Standards.
The best International Oil Industry practice and relevant goal setting legislation have been
adopted to reduce the level of risk to as low as reasonably practicable and therefore well below
that mandated by applicable statutory laws and regulations.
In the absence of local regulations, BP Group Standards will apply. In addition, appropriate UK
and US regulations and industry best practice have been considered in setting suitable goals
and targets.
1.4
Safe System of Working
An SSoW is a process that is entirely competent person dependant. It takes a request to
perform an activity and after challenging it through the system, formally gives approval to
proceed under stated controls. The System systematically identifies and controls any hazards
that may be present in the task, in the environment in which the task is to be performed and in
any hazardous interactions with other activities in the same area.
The Safe System of Work starts with Planning in order that all activities in an area, whether

persons for the area so that a suitable Assessment of Risk may be carried out to identify any
hazards and allocate appropriate control measures. Should the activity require the application of
company standards, these should be consulted to ensure compliance. The activities must be

co-ordinated to prevent any conflict between work and operational tasks. Supervision must
identify and control hazards, authorised work to proceed, and ensure continued compliance with
conditions during work period.
The suitable and sufficient Assessment of Risk is an essential part of the Safe System of Work.
Assessment of risk can generally be achieved by the Permit to Work system. In instances where
there may be a higher risk potential in the task, or compliance with Company Standards cannot
be achieved, Risk Assessment level 2 can be triggered. Risk Assessment level 2 is a formalised
team process for the assessment and control of risk (See AZSPU-HSSE-DOC-00063-2).
The Permit to Work System is a formal written system used to control work. It communicates
requirements between site / installation management, plant supervision, operators and those
who carry out the work, and anyone involved in the safe system of work. Essential features of
the system are:

While the Permit to Work System, Company Standards and Risk Assessment level 2 provide for
Safe Systems of Work, the role of the individuals within the system cannot be understated. Each
must exercise their competence and discharge their responsibility in support of the system.
1.5
Company Requirements
It is a company requirement that all tasks are subjected to an assessment of risk to demonstrate
that risks have been reduced to as low a level as reasonably practicable (ALARP). This can be
achieved by complying with the Company‟s existing standards. Where compliance with
Company standards cannot reasonably be achieved, a formal level 2 Risk Assessment will be
undertaken to identify any additional controls and demonstrate that risks remain as low as
reasonably practicable. Whether by compliance with Company Standards or through level 2
Risk Assessment, the Company‟s Golden Rules of Safety must be complied with. Golden Rules
are non-negotiable.
1.6
Stopping Unsafe Work
To stop the continuation of potentially unsafe work at the earliest possible stage the Control of
Work (CoW) Policy and this Permit to Work procedure make it very clear that all personnel are
obliged and have the authority to “STOP” the work that they consider to be unsafe.
1.7
Deviations & Local Rules
This procedure is written in sufficient detail that it should be able to be applied consistently at all
sites or installations. There may still be the requirement for some local rules covering site or
installation specific logistical & administrative arrangements and local variations in
responsibilities to reflect differences in organisational arrangements. These local rules should
not deviate from the core processes within this document. Any form of deviation from this
procedure, including but not limited to local rules, shall be requested and authorised in
accordance with the SSOW Deviations from Regulations and Procedures (Doc. No: AZSPU-
HSSE-DOC-00011-2).

1.10 Permit to Work Golden Rules of Safety
PTW is one of BP‟s Golden Rules of Safety and states:
Before conducting work that involves confined space entry, work on energy system, and ground
disturbance in locations where buried hazards may exist, or hot work in potentially explosive
environments, a permit must be obtained that:
 Defines scope of work
 Identifies hazards and assesses risk
 Establishes control measures to eliminate hazards or mitigate the risk
 Links the work to other associated work permits or simultaneous operations
 Is authorised by the responsible person(s)
 Communicates above information to all involved in the work
 Ensure adequate control over the return to normal operations
1.11 Language Facilitation
Due to the various languages spoken at site, there is a necessity to assist all with “an ease of
understanding”. Therefore, the development and use of information tools are available.
1.12 Procedure Summary
A Procedure Summary has been developed in a form of a leaflet, which can be carried by the
Line Supervisors while conducting their day-to-day work tasks. The Leaflet summarizes the
contents of this Permit to Work procedure. The Procedure Summary can also be used as a
guideline for Line Supervisors to deliver their daily toolbox talk. (See Appendix 14)

3 ROLES & RESPONSIBILITIES
The key roles and responsibilities within the PTW process are described below.
3.1
Performance Unit Leaders (PUL)
Performance Unit Leaders are responsible for:
 Ensuring that the PTW Process is applied at sites within their area of responsibility.

 Periodic internal reviews and / or audit of the operations of the PTW.
3.2
Asset Manager
The Asset Managers are responsible for:
 Ensuring that the PTW Process applied at their sites are authorised by them prior to
implementation.
 Periodic self-regulatory reviews.
3.3
Site Managers (SM) / Site Controllers (SC) / Offshore Installation Managers (OIM)
Site Managers Site Controllers and Offshore Installation Managers are responsible for:
 Overall operation of the PTW on their site and ensuring that the procedures described in
this document are consistently followed.
 Ensuring that the PTW process is subject to regular monitoring and auditing, acting
upon the results of these audits to maintain the integrity of the system and proposing
any recommendations for system improvement.
 Ensuring that the training and competency standards, as defined in this document, are
followed and to satisfy him / herself that the AA is competent
 Authorization of all categories of Work Permits.
 Approval of all Level 2 Risk Assessments (Normal, ORA, IRA & SARA).
 Approval of lessons learned and audits.
 Signature for approval of Deviations from this procedure.
 Approval of all Formal Procedures
3.4
Department Head (DH)
The Department Head (where applicable) roles and responsibilities are:
 Operation of the PTW process within their areas of responsibility.
 Countersignature of all categories of permit within their area of responsibility and
ensuring that the appropriate hazards and controls have been identified and mitigations
are in place for the planned task.
3.5
Area Authority (AA)
The AA is responsible for the day-to-day management of the PTW process within their area of
responsibility. The AA is normally the Shift/Operations Team Leader or equivalent, although any
individual can be dedicated to the role providing the individual is trained and competent. There
may be more than one AA at any particular site. The duties of the AA are:
 To report to the SM/SC/OIM and have overall responsibility for the safe control of work
activities in accordance with these procedures and within their designated area. This
includes the issue of all Work Permits.
 Liaising closely with the PA‟s when planning permits, to ensure that the appropriate
hazards and controls have been identified for that task.
 Ensuring that the appropriate level of risk assessment has been carried out for the task
(Level 1 or 2) and acting as the Task Risk Assessment Team Leader
 Ensuring that all the appropriate control measures have been put in place prior to a
permit being issued, confirming that the PA fully understands the scope of the task.
 Providing the culture to “STOP the Job” if anyone feels unsafe.
 Approval of isolation design, control of isolation implementation and ensuring that the
isolation is in place prior to allowing an associated permit to be issued. Also ensuring
that the isolation is properly removed after completion of the work and cancellation of
the permit.

 To ensure that the worksite inspections are carried out before, during and after the
performance of each task (some of this task activity may be delegated to a competent
nominated person).
 To ensure that there is a walk through of every work site activity before and after
completion of work as a minimum, ensuring good housekeeping, isolations and tags
removed as appropriate.
 Ensuring that adequate handovers take place at shift change, crew change or other
change out/over of AA‟s, PA‟s and IA‟s
 Validate lessons learned and audits.
 To ensure that any cancelled permit to work documents are replaced with new ones.
 Be familiar with all AzSPU Caspian SSOW documents.
 Revalidation of Permits
3.6
Affected Area Authority (AAA)
This applies where there is more than one AA on a site. Where activities carried out in one area
impinge or impact on activities in another area. (e.g.: Simultaneous Operations-SIMOPs), then
the AA must inform the AAA. The AAA must:
 Be aware of all responsibilities of the AA as above.
 Countersign the Permit from the adjoining area to confirm that he/she is aware of the
activity-taking place and that the hazards can be effectively managed with specified
controls.
 Be aware about duration and types of all isolations affecting the work area and the area
under his/her responsibility.
 Communicate with personnel working within his/her area that may be affected by the
adjacent activities to ensure that they understand the potential impact on their activities.
3.7
Performing Authority (PA)
The PA is the responsible person for the activity being carried out under the Permit. The PA
must be competent to perform the task and be fully conversant with the equipment and tools
being used. The PA may be the person carrying out the task or may be supervising a group of
people carrying out the job. The PA can be responsible for more than one task at any one time
providing he/she can safely manage the tasks concurrently. The PA‟s main duties are to:
 Reports and interacts regularly with the AA and AAA on any Management of Change
(MOC) issues to ensure the risks from all hazards are mitigated by controls to ALARP.
 Create the Permit and identify the hazards and control measures
(Level
1 Risk
Assessment) for the task being planned.
 Participate in any Level 2 Risk Assessment where required.
 Ensure that where other persons are involved in the task, they fully understand the
scope of the work and the hazards and controls for the job by holding a toolbox talk
meeting. This includes ensuring that all of those involved in the specific work activity
sign off the worksite hard copy of the Permit.
 Provide the culture to “STOP the Job” if anyone feels unsafe.
 Ensure that only personnel authorised by the Permit participate in the work and no
unauthorised interference takes place.
 Ensure that all controls are applied prior to commencing task.
 Ensure that only work covered within the scope of the Permit takes place.
 Ensure that lessons learned from the job are captured.
 Ensure that where there are any deviations from the initial Permit conditions the work
will be stopped and reassessed.
 Ensure that the worksite is kept in a clean and safe condition both during and upon
completion of the job.

 Ensure adequate handovers take place at shift and crew change periods with the
oncoming PA and AA.
Note: A PA and an AA cannot be the same person on a permit. (e.g.: each task has to have
separate PA‟s and AA‟s)
3.8
Control Room Operator (CRO)
The role of the Control Room Operator (CRO) varies considerably between sites and another
person, typically an AA or AAA, may carry out some of these duties. Inhibition and
reinstatement of sections of the fire and gas detection or protection systems in support of work
control activities in accordance with requests made by the AA. The duties of the CRO within the
PTW system are, but not limited to:
 Ensuring safety system status board is up to date.
 Ensuring inhibit / override register is kept up to date and audited.
 Ensuring all inhibits / overrides are handed over at the end of each shift.
3.9
Authorised Gas Tester (AGT)
Authorised Gas Testers are approved persons who have been trained and certified in gas
testing, they are authorised to test for the presence of flammable vapours, toxic gas and
oxygen as required in support of the Permit or Entry Certificate as requested by the AA.
Level 1 AGT’s are competent to carry out gas testing on all activities including Confined Space
Entry (CSE) activities. For confined space work the Level 1 AGT must retest the atmosphere at
the start of each shift, or when the work has been suspended.
Level 2 AGT’s are qualified to carry out gas tests in support of all activities excluding
Confined Space Entry & carry out continuous monitoring during ongoing work.
Level 3 AGT’s are individuals, usually the PA, who is approved by the Site Controller as having
undergone practical instruction by a Level 1 or 2 AGT on the use and interpretation of the
results from both portable and personal gas monitors. The AGT3 has no authority to record gas
test results on PTW; their responsibility is only for continuous monitoring.
3.10 Firewatcher (FW)
A firewatcher must be present to monitor the work area while fire and gas systems are inhibited
for hot work, e.g. welding and burning. The Firewatcher must be suitably trained in the
equipment they have to handle and are responsible for ensuring that:
 Suitable fire fighting equipment is available and ready for immediate use.
 Flammable materials are cleared away from the worksite.
 Drains remain covered and sealed.
 Sparks and welding spatter are contained (by the use of fire blankets, water sprays etc)
 They are familiar with the location of the nearest fire alarm activation means and when
and how the fire alarm will be raised where a fire or gas release occurs in the area.
 The alarm is raised should there be a fire or gas release in the area.
 In complex multi-deck layouts, more than one firewatcher may be required.
3.11 Isolating Authority (IA)
The Isolating Authority is responsible for isolating specific sections of plant or items of
equipment to the highest quality and for the security of isolation, which is reasonably
practicable. The IA is also responsible for demonstrating the integrity of the isolation to the AA
and PA and for monitoring the integrity of isolations whilst they are in force. The relevant IA shall
also witness the insertion of spades to achieve positive isolation when required. The IA will be

responsible for ensuring compliance with energy isolation requirements. The IA can also be the
PA if required.
3.12 Responsible Electrical Person (REP)
The Responsible Electrical Person shall have a clear overview of the installation and of all the
electrical work being carried out. The REP will approve any switching programs, will
countersign permits with electrical content where the AA isn‟t electrically competent, ensuring
that the isolation has been correctly designed and the Isolating Authority has the appropriate
authorisation level for the work. The REP will be an Electrical Authorised person having the
highest level of authorisation required for the site being worked on. At any time there can only
be one REP for each installation.
3.13 Permit To Work Co-ordinators (PTWC)
PTW Co-ordinators are required to operate, monitor and control the Permit System. The Permit
to Work Coordinators main responsibilities will be but not limited to:
 Issuing and subsequent control of the PTW system on the site they are allocated.
 Routine site tours monitoring PTW application.
 Ensuring the correct permit and and/or isolation certificate is used.
 Ensuring the correct signs, tags and locks are used.
 Act as focal point for all matters relating to Permit Issue and Control.
 Attend work planning and scheduling meetings.
 Attend PTW & Risk assessment meetings
 Carry out regular audits on the PTW system.
 Maintaining the Long-Term Isolations
(LTI) Register and carrying out audits as
specified.
4 COMPETENCY, TRAINING & AWARENESS
4.1
General
All personnel involved in the use of the PTW shall be both trained and proven to be competent
to the appropriate level. The competency of all relevant personnel shall be established during
the planning process for a particular job. The AA and/or Line Supervisor shall ensure that the
personnel involved in the activity have the correct competencies through records or requesting
individuals to produce relevant certification.
4.2
Levels of Training
4.2.1 First time users
Performing Authority level using classroom based training, with possibly a computer-
based Training
(CBT) for refreshers. This also requires a practical field based
assessment to demonstrate that the trainee understands the Permit to Work system.
Area Authority Personnel training for the first time at AA level shall attend the AzSPU
approved training in accordance with the Authorization procedure AZSPU-HSSE-DOC-
00012-2
PA‟s and AA‟s can only take on the role when they have been authorized by the
SM/SC/OIM.
4.2.2 for those considered experienced
(Regular users) Refresher training shall be undertaken, and assessed, at a frequency of
not more than 2 years, or if absent from the operation for more than 12 months.

4.2.3 for those transitioning from another PTW system to this one
Transition training shall be undertaken with all attendees being assessed for
competency acceptance.
4.2.4 All AzSPU employees
Prior to any involvement with the PTW system all employees shall receive awareness
training to ensure that they understand the importance of PTW and how it affects them
in the working environment.
4.3
Records
Records of attendance and competency will be filed for future reference / verification by
SM/SC/OIM.
5 AUDITING & MONITORING
All assets shall:
 Undertake internal audits of the operation of the Permit to Work System at each site.
 Maintain an Audit Register.
 Have in place a system for tracking recommendations through to close-out.
 Develop an audit matrix to ensure that audits are a cross section of various ongoing
activities, i.e. hot work, cold work, spark potential & cancelled permits.
 Use of a Standard Audit Checklist is recommended, to allow comparison with external
audit results. (See Appendix 11 & 12)
The recommended frequency of Audit is as follows:
 Supervisor/ Area Authority Minimum of 2 permits per Week
 Site Safety Adviser
Minimum of 1 permit per week
 Site Manager/ Controller
Minimum of 1 permit per month
SM‟s, SC‟s and OIM‟s shall carry out regular internal reviews of the findings of Permit to Work
Audits to ensure that any critical failings in the system, or its manner of implementation, have
been identified and appropriate actions have been taken.
6 THE PERMIT TO WORK SYSTEM
6.1

Hot Work Naked Flame (See Appendix 1)
Permit for Hot Work could involve any of the following activities:
 Naked flames (welding, flame cutting)
 Electrical welding
 Electrical induction pre-heating, stress relieving or use of high temperature thermal
calibrators (above 200°C), except in authorised workshops
 Use of portable grinders (air or electrically powered)
 Abrasive wheels
 Use of flare guns
 Use of fibre optic fusion splicing equipment in hazardous zones.
 Use of heat shrink blowers in hazardous zones.
Note: A HWNF Permit is not required for operations and/or maintenance activities
involving ignited gas flares or permanently mounted plant using an enclosed flame
(boilers, inert gas generators, etc).
6.3
Hot Work Spark Potential (See Appendix 2)
Permit for Hot Work Spark Potential could involve any of the following activities:
 Dry grit/shot blasting.
 Use of heat shrink blowers in non hazardous zones.
 Use of fibre optic fusion splicing equipment in non hazardous zones.
 Needle gunning.
 Pneumatic chisels
 Use of non-appropriately rated EX equipment
 Opening live electrical junction boxes where the terminals are exposed to atmosphere

 Use of air or hydraulically powered tools, mechanically capable of generating a spark
 Use of electrically powered equipment capable of generating a spark
 Work involving explosives and perforation guns
 Use of cartridge operated fixing tools
 Use of various vehicles out with the use on normal roads
 Operation of protected portable diesel engines not tied into fire and gas systems.
Note: It is Company policy to avoid hot work in hazardous areas wherever practicable. It
is the role of engineers planning the work to minimise the need for hot work and
provide cost effective alternatives by careful consideration during the design and
planning phase.
6.4
Cold Work Breaking of Containment (See Appendix 3)
Permit for Cold Work (Breaking of Containment) could involve any of the following activities:
 Opening up of any process system where there is a risk from egress of toxic, flammable
or otherwise hazardous substances.
 Construction, maintenance, overhaul, repair work involving breaking containment on
flammable or otherwise hazardous process systems in operational areas.
 Spading and de-spading of systems which contain toxic, flammable or hazardous
substances.
 Sampling of Hydrocarbon products by any means other than an approved sample point.
Note: Topping up, refilling equipment or process systems, handling or exposure to
flammable equipment does not require a CWBC permit as long as all hazards and
controls that are identified on the COSHH / PTW are in place.
6.5
Cold Work (See Appendix 4)
Permits for Cold Work could involve any of the following activities:
 Working on vessels/equipment contaminated with Low Specific Activity (LSA)
scale/Naturally Occurring Radioactive Materials (NORM)
 Working with radioactive sources
 Working with asbestos or mineral fibre products
 Civil and ground preparation works
 High pressure water jetting or wet grit blasting
 Ultra High Pressure (UHP) water cutting
 Painting/spray painting
 Removal of handrails, gratings, hatches and fixed ladders
 Scaffolding erection/dismantling
 Use of lasers
 Work affecting the availability of fire and gas detection systems
 Work affecting the availability of fire or explosion control or protection arrangements e.g.
deluge, fixed fire fighting, fire pumps, fire main etc.
There could be instances where specialist tools or equipment are required that are not specified
in the above lists. These instances should be risk assessed and the category of permit used
should be decided upon the findings of the risk assessment.
6.6
Confined Space Entry (See Appendix 11)
A Confined Space Entry permit shall be raised when it is necessary for personnel to enter
confined spaces as defined in Doc. No: AZSPU-HSSE-DOC-00013-2 Confined Space Entry. A
confined space is a place which is substantially, though not always entirely, enclosed and where
serious injury can occur from hazardous substances or conditions within the space or nearby.

 Tanks, vessels, separators,
 Silos, ducts, sewers, pits, flues,
 Manholes
 Voids between modules and in legs on offshore Installations.
 It may also include any space in which dangerous levels of contaminants can
accumulate and ventilation is restricted, e.g. excavations deeper than 1.2m, the space
above floating roofs on floating roof tanks, open-topped tanks, closed or unventilated
rooms, sumps and culverts, and any other poorly ventilated areas.
The hazards that this document addresses arise through the confined nature of the place of
work in combination with one or more of the following:
 The possible presence of flammable and/ or toxic substances
 An oxygen-deficient atmosphere
 An oxygen-enriched atmosphere
 Ingress of solids or liquids
 The presence of excessive heat
The Confined Space Entry permit requires the following criteria to be followed:
 It can only be issued to an AGT Level 1
 A Level 2 Risk Assessment is mandatory
 It does “not” permit “any” form of work, only visual inspection and gas testing
 Only one confined space to be allowed on each certificate.
The Confined space entry permit ensures the following activities:
 Declaring that (where applicable), the confined space is positively isolated under formal
ICC so that the AGT1 can enter, subject to any special conditions.
 Authorising entry by the AGT1
 Recording the re-test results in section 8. (Provision is made for gas tests in Section 8,
including a continuation sheet).
 Declaring the confined space safe for entry.
 Specifying whether or not Breathing Apparatus is required.
 Written & documented communication between the AGT1 and AA.
6.7
Formal Procedure
Certain activities do not normally need to be covered by a Permit within the SSOW process.
Competent people using approved formal procedures may carry out these tasks. Where
situations change then it is important that these procedures are reviewed and updated
accordingly.
Formal Procedures are authorized by the Operations team leader & Site Controller and are
reviewed annually. A Formal Procedure is valid for 1 shift (12 hours). A Formal Procedure-
Application to perform work is used to request this work-scope. The register of valid Formal
Procedures is held by the Operations support team leader and should be clearly displayed in
the permit office to be accessed by the AA‟s and PA‟s.
These activities may include the following Operations carried out in accordance with Installation
procedures covering the following unless otherwise agreed:

within their respective PTW systems.
6.8
Work not requiring a Permit or Formal Procedure (Non-Permitted Work)
Certain activities do not normally need to be covered by a permit within the SSOW process.
These activities may include the following:
 Production plant operations as clearly described in formal operating / CMAS procedures.
 Routine offshore crane operations.
 Routine Drilling Operations
 Use of the following tools and equipment inside accommodation areas, workshops,
control rooms and other non-hazardous modules protected by fire and gas detection
equipment:
o Battery operated cameras without flash
o Processes involving naked flames or hazardous substances in approved laboratories
 Visual inspection of areas (except confined space and rope access).
 Operation of equipment for approved training purposes e.g. use of fire fighting or life
saving appliances during drills.
 The handling and use of non hazardous materials.
 All Work other than HWNF, carried out by competent technicians (i.e. machining, fitting,
turning and calibration work) in workshops in Non-Hazardous areas.
6.9
Types of supplementary certificates
Four supplementary certificates
(see appendices for formats) are provided for use where
activities have to be performed before a Permit to Work can be issued:
 Isolation Confirmation Certificate
 Plant Contamination Certificate
 Clearance for Excavation Certificate
 Clearance to Move Heavy Equipment Certificate
6.10 Isolation Confirmation Certificate (See Appendix 6)
Where items of equipment require being isolated to allow the work to take place safely then an
Isolation Confirmation Certificate (ICC) must be raised to control the isolation except in the case
where a personal isolation is acceptable (see section 9.6).
The Isolation Confirmation Certificate applies to all types of isolation, covering process, control
and electrical. The ICC contains a listing of all isolation points and the AA must approve the

design before it can be applied. Individual isolation points must be signed off by the Isolating
Authority (IA) to confirm they have been put in place. Only on confirmation by the AA that all
isolations are in place, can the associated permits be issued. The ICC must remain in force until
all permits associated with the ICC have been cancelled. Any Sanction to Test
(STT)
requirements should be specified at the time of creation of the ICC where this is known,
although they can be identified at a later stage.
The Isolation Confirmation Certificate supports the Permit to Work by providing the means of:
 Recording the isolations which are required before the task detailed on the associated
Permit to Work can proceed
 Confirming isolations have been made so that the task can proceed
(subject to
authorisation of other certificates e.g., Confined Space Entry)
 Authorisation and recording of de-isolations and isolations which may be required to test
equipment under a sanction to test
 Authorisation and recording of de-isolation on completion of the task detailed on the
associated Permit to Work.
Note: All requested isolations require the completion of The Isolation Confirmation Certificate
duly signed by the IA before the relevant Permit to Work can be issued to the PA.
6.11 Plant Contamination Certificate (See Appendix 7)
A Plant Contamination Certificate shall be used to cover the handling or transport of equipment,
which is, or has been, contaminated.
The certificate provides the means of:
 Declaring that a contaminated piece of equipment has been cleaned, specifying the
method(s) used.
 Defining the substances with which a piece of equipment is contaminated if it has not
been cleaned.
 Specifying the precautions to be taken when handling a piece of contaminated
equipment.
The certificate shall be securely attached to the equipment and a copy included with the
manifest if the equipment is to be transported from the installation / site.
6.12 Clearance for Excavation Certificate (See Appendix 8)
A Clearance for Excavation Certificate shall be used where any excavation or stake driving is
planned, on any site.
The certificate provides the means of:
 Applying for permission to excavate as specified.
 Recording the consent of those engineers responsible for the technical integrity of any
underground equipment or services, subject to any specified precautions.
 Recording the consent of the AA, subject to the issue of a Permit to Work.
 Acceptance by the PA, and his undertaking to observe the required precautions and to
obtain the necessary Permit to Work before starting excavation.
Note: Only one excavation per certificate is allowed, clearly defining the boundaries of
the excavation.
6.13 Clearance to Move Heavy Equipment (See Appendix 9)
A Clearance to Move Heavy Equipment Certificate shall be used when it is planned to move
heavy equipment that potentially could damage underground, overhead, surface and surface
mounted services and facilities. The certificate provides the means of:

 Specifying the precautions to be taken.
 Recording the consent of the AA subject to the issue of a Permit to Work.
 Acceptance by the PA, and his undertaking to observe the required precautions and to
obtain the necessary Permit to Work before starting the move.

PROCEDURE FOR COMPLETING A PERMIT TO WORK CERTIFICATE
Completion of Section 1 - Task Description
This section is completed by the person requesting the work or his representative. The
information can be extracted from a detailed work scope. Details of the task on separate sheets
and drawings should be acknowledged in this section. These sheets and drawings should be
attached to the Permit.
The type of Permit to be issued must be selected at this stage. In cases of doubt, the person
initiating the Permit should consult the AA. The person initiating the Permit must also indicate
the following:
 Exact Location of the work (defined as accurately as possible and quoting equipment or
line numbers where relevant).
 Equipment to be worked on (including Tag / Equipment numbers).
 Exact nature of the work to be undertaken.
 Tools and equipment to be used, paying close attention and thought for tools and
equipment that may introduce hazards out with performing the task.
 Any Maximo references & Work Orders.
The person requesting the work endorses the work specification with his name, printed and
signed.
Completion of Section 2 - Risk Assessment
This section specifies the hazards that are likely to be encountered by persons engaged in the
work. These hazards will result from the task being performed, the environment in which the
work is being performed, the tools and equipment being used in the task and from any Adjacent
or Associated Activities. The section is completed by the PA and checked & discussed with the
AA.
Once the hazards have been identified, appropriate precautions to be taken must be listed this
is also completed by the PA and checked & discussed with the AA. The requirement for
Supplementary Certificates will also be identified in this section. Any Supplementary Certificates
raised will be cross-referenced to Section 3. Tasks which are adjacent to or associated with the
proposed work are listed in this section.
The PA or AA may consider the proposed task to be of such an unusual or high risk nature that
he considers a formal Level 2 Risk Assessment is called for. In such a case the task will be
postponed until the Assessment is completed. A reference to the Level 2 Risk Assessment will
be made on the Permit. Precautions to be taken on the plant/equipment and its immediate
vicinity must be specified.
A site inspection by the AA or his delegate accompanied by the PA is a requirement prior to
issue of all permits being issued. Inhibitions to the Fire and Gas Detection System and
Shutdown System must be detailed.

The PA must specify the type of protective clothing and equipment that the personnel carrying
out the work must have available and use. The protective clothing and equipment is that in
excess of the general requirement for the site, i.e. safety glasses, coveralls, work boots, hard
hat need not be specified. Precautions to be taken at the worksite must be specified
(emergency equipment and personnel, limitations on the use of equipment, and other special
precautions).
The requirement for continuous gas monitoring by the PA / AGT 3 can be specified. The type
and number of detectors will depend upon the likely source(s) of gas or vapour, the type of work
and local conditions (e.g. wind conditions, ventilation arrangements, etc.).
When a written instruction/procedure is specified, a copy should be made available for the PA's
perusal, i.e. LSA scale, H2S, Radiography etc. On all permits the concession to carry out
Personal Isolation/De-isolation (see section 9.6) is indicated in this section.
Completion of Section 3 - Cross referenced certificates
This section cross-references Supplementary Certificates for specialist activities supporting the
Permit. They are recorded by the AA. If personal isolation is approved (see section 9.6), the AA
must state “personal isolations” in this section.
Completion of Section 4 - Sanction to test
This section contains the Sanction to Test authorisation. This is completed by the AA before any
test requiring changes in isolations can proceed. To allow equipment to be tested while the
Permit and ICC are still in operation, the signatures of the Area and Isolating Authorities must
be obtained. The signatures indicate consideration has been given to others possibly working
on the equipment, change of isolations required and the controls required for the test to
proceed. On the ICC, amendments to the isolations are documented and initialled by the Area
and Isolating Authority. On the permit, the AA signs the Sanction to Test declaration authorising
the test to proceed. When the test is completed the same authorities resign the ICC and permit
indicating the restoration of the isolations and authorising work to proceed. This procedure can
be repeated until a satisfactory test has been achieved, or until this section has been completely
used, in which case a new Permit will be raised.
Note: Where personal isolation has been granted on Permits, de-isolation for test purposes will
be permitted by the PA without recourse to the AA, provided that testing has been
specified in Section 1 of the Permit (Specification of Work).
Completion of Section 5 - Authorisation
At the daily permit control & planning meeting all new permit applications are discussed by all
AA‟s, affected AA‟s are identified, and all adjacent activities are taken into consideration.
Potential conflicts with existing permits are highlighted. It is at this meeting that the site
controller will authorise all new permits. (See section 9.5 for more details)
Completion of Section 6 - Site gas tests
This section on the Hot Work Naked Flame Permit is mandatory and is completed by an
Authorised Gas Tester Level 1 or 2. For Naked Flame work in a non-hazardous area, the area
must be cleared of flammable and combustible materials, and a flammable gas check made
before the Permit is issued. Unless there is reason to suspect that conditions may have
changed, there is no need to retest.
For Naked Flame work in a hazardous area, the same initial precautions as above are
necessary. As flammable gas or vapour may subsequently be released into the work area,
portable continuous gas monitors must be utilised as specified in Section 2 of the Permit
Retests are required at revalidation and before every working period.

Completion of Section 7 - Issue
Prior to issuing any permit, the AA carries out a worksite inspection accompanied by the PA,
showing him the points of isolation and other potential hazards. The Live permit listing from the
PTW register or the permit location board should be reviewed by the AA at this point to identify
and control any conflicting permits. Any necessary inhibition of the Fire and Gas Detection
System or Emergency Shutdown System as specified in Section 2 is carried out by the CRO. To
minimise the time for which any area is unprotected, work should not be delayed once the Fire
and Gas Detection System has been inhibited. Inhibition of these systems should only be
carried out immediately before the Permit is issued for work to commence. Following these
checks the AA and the PA both sign this section.
Completion of Registry 8 - Permit registry
Should there be no conflicts; the PTWC will then sign the permit. Once the top copy is displayed
at the worksite, the task may now commence.
Note: Permits should only be registered for work that will start immediately.
Completion of Section 9- Re-issue and return
A Permit can be reissued in accordance with the following procedure, up to the expiry date.
Unless immediate revalidation is required, any inhibitions of the Fire and Gas Detection or
Shutdown Systems should be removed and then remade immediately prior to reissue (e.g. next
morning). This ensures that inhibits are in place are in place only while necessary. Also:
 The top two copies of the Permit must be signed by the relevant persons for each renewal
 Results of gas tests (etc) when applicable must be logged on the Gas Test Section and
signed by the person carrying out the test.
 All inhibitions of the Fire and Gas Detection System must be completed before reissue of
the Permit
 Should there be no conflicts, the AA / AAA and the PA both sign this section.
 All reissued Permits must be registered in the PTW register.
Completion of Section 10 - Cancellation
Performing Authority: When the work has been completed or cannot be continued, all copies
of the permit must be collated in the Permit office. The PA signs the declaration in Section 10
and specifies whether the work is complete or incomplete. When work is incomplete a brief
description of the reason and equipment state should be made.
Area Authority: When the PA has signed the first part of Section 10, the AA or his delegate will
inspect the worksite. The AA satisfies himself that the work has been properly performed, that
the area and plant are clear of all tools and equipment and the site is in a safe condition. He will
ensure that any referenced ICC‟s are removed then complete the declaration accordingly. He
signs this section to cancel the Permit.
Note: When work in completion necessitates prolonged isolations, e.g. awaiting spares etc.,
then a record must be kept of the equipment state. This could be a notation in the control room
shift log or a separate detailed sheet explaining the reason for the Long Term Isolation.
Additionally one copy of the now cancelled Permit should be attached to the covering ICC.
Completion of Section 11 - Registry of work completion
When all copies of the PTW have been signed and returned to the Permit office, the Fire and
Gas Detection System and Shutdown System can be reinstated. The top copy of the cancelled
Work Permit is retained for one year on site as material for external auditing.

PROCEDURE FOR COMPLETING AN ISOLATION CONFIRMATION CERTIFICATE
8.1
Completing an ICC
The control section of the ICC is completed by the AA. He indicates the specific item of
plant/system that is to be isolated, stating equipment, tag and system numbers wherever
possible. When drawings, blanking schedules or P&IDs are available detailing the isolations
required the appropriate tick box is completed. A precise reason for isolation, including clear
indication of any requirement for Hot Work, Confined Space Entry or Mothballing, along with a
realistic estimation of the duration of the isolation, enables the appropriate isolation standard to
be applied. Should the reason for isolation escalate beyond that stated at the outset, a review of
the suitability of the isolation standard must be carried out, and appropriate action taken.
Completion of Isolation Control Section
The AA ensures that the plant/system is safe for isolations to be applied. This could involve
draining, depressurising, venting a system, etc, or arranging for standby units to be set up for
operation, etc. The AA indicates the means of isolation (Valve, Positive, Instrument or Electrical)
that will be required and then signs the declaration in the Isolation Request Section.
Completion of Isolations Record
This section of the ICC records all applied items of isolation. Under the appropriate discipline
heading each point of isolation is recorded. It is described, quoting item numbers if available.
How the item is isolated is detailed by completing the appropriate boxes with Yes/No. NOT
TICKS OR CROSSES.
Note that the box titles differ between the valve/positive isolations and the electrical isolations.
The box should confirm each isolation is labelled and secured. Lock key number should be
recorded. When alternative means of immobilisation are employed, and no unique lock number
is available, the item tag number, or label number can be recorded. The Isolating Authority must
sign his name for each isolation he has applied. When attached documents contain the list of
applied isolations the Listing should be attached to the ICC and the ICC should clearly reference
the listing.
Isolation Confirmation Certificate Registry
The completed Certificate is given to the PTWC. The ICC details are recorded in the Permit
Register and the Registry Number recorded. The PTWC signs the ICC, all continuation sheets,
attached drawing, blanking lists, P&ID‟s which are signed by the Isolating and AA‟s as part of
the ICC. When multiple sheets are involved they would be indicated as Sheet X of X. The both
copies of the ICC are retained in the permit office.
Record of Connected Permits to Work
A number of Permits to Work may be raised against an ICC. These connected Permits to Work
must be recorded, as they arise, on the appropriate section of the ICC. The facility is meant to
be an aide memoir to the AA. As permits are cancelled he will sign off the appropriate Permit
Number, keeping this list of connected Permits up-to-date.
Note: The Positive isolations on the ICC will generally be applied or removed under the control
of a Permit to Work. The work is being carried out on the strength of the „Partly
complete‟ ICC. The permit to apply or remove Positive Isolations must be cross-
referenced to the ICC.
Permits issued for the activity of applying positive isolation are cross-referenced in this section.
This activity is being carried out against valve isolation for the purpose of completing the
isolations for the main work. The AA must pay particular attention to controlling the hazards
involved in breaking containment for applying positive isolation.

Record of overlapping ICCs
A number of ICC‟s may share common isolations. These isolation points would carry a label for
each ICC on which they are recorded. These areas of overlap between ICC‟s should be
recorded on each ICC as an aide memoir to the AA.
Completion of De-isolation Control Section
When it is required to remove all the isolations presently applied under the ICC, the AA will sign
this section of the ICC. He signs to declare that ALL Permits to Work and Entry Certificates
connected to the ICC have been CANCELLED, and that all work carried out under this ICC is
COMPLETE. Only when he is fully satisfied that the worksite is safe will he sign the de-isolation
request.
When signed, the relevant Isolating Authorities proceed to remove the isolations under the AA‟s
instruction. As they are removed the Isolating Authorities confirm the de-isolation by signing
their names opposite each item in the de-isolation signature column of the ICC.
When drawings, blanking lists, P&IDs, etc, have been used as part of the ICC documentation,
each item must be signed as de-isolated when the isolations are removed. When work against
the ICC has stopped, the permits have been cancelled, but the work is not necessarily
complete, the ICC will have to remain live as a Long Term Isolation. The AA records the reason
for retention of the isolations, and gives an estimation of the time until the work can be restarted
(i.e. 3 weeks wait for spares). He then indicates what he has done to secure the isolation for this
extended period. This may involve enhancing the isolation standard applied.
Return to Normal Operations Declaration
When the AA has confirmed that all isolations have been removed, a de-isolation signature
having been given opposite each item of isolation, he signs for the plant to be returned to
normal operations. He also confirms the completion, if any requirement for Leak Testing has
been fulfilled. This section records that the isolated plant/system is now available to return to
normal operations.
Completion of Registry of ICC Cancellation
When the ICC have been returned to the Permit office, duly signed by the Isolating Authorities
and the AA to signify that the plant/system has been de-isolated and is now available for
normal operations, the PTWC will cancel the ICC. The PTWC makes an entry in the Permit
Register that the ICC is now cancelled and signs the Registry of ICC Cancellation section of the
ICC. The ICC and all associated documentation are retained for one year on site as material for
external auditing.
Isolation Confirmation Certificate Continuation Sheet
The ICC is a multi- discipline document. It is appreciated that when plant/system is isolated
there may be disproportionate numbers of isolations from the different disciplines. When the
total number of isolations exceeds the capacity of a section of the ICC form, a continuation
sheet may be used. There are two types of Continuation Sheet. One carries the headings for
Electrical Isolations; the other carries the headings for Valves/Positive Isolations. The details of
the isolation are recorded as on the main ICC. The Registry Number of the main ICC is carried
on the continuation sheet also. A notation of is made to advise the PTWC of the total
documentation involved. The bottom of the continuation sheet contains additional boxes for
recording Connected Permits.
8.2
Procedure for Obtaining a Sanction to Test (STT)
Certain work requires the testing or running of equipment prior to completion of the job and full
removal of the isolation. Where this is required a STT is requested. This may require the
removal or partial removal of the isolations to allow test running of equipment before the total
scope of work is complete. This operation must be tightly controlled.

The isolation points required to be temporarily de-isolated must be reviewed with the AA and IA
and then the Associated PTW affected by the STT should be suspended. Once the AA has
given approval for the STT, the IA can de-isolate the relevant isolation points. Once the initial
de-isolation has taken place then the associated PTW which was withdrawn can potentially be

On completion of the test and before any work can recommence under the original PTW, each
point of Isolation must be returned to isolated state, and all electrical items that were originally
proved dead must be proved dead once again. This approval, de-isolation and subsequent re-
isolation must be recorded on the ICC.
If following a STT a successful test has been achieved and this marks the end of the scope of
the task & permit, and provided all other connected permits are signed off as complete, then
these isolation points do not need to be re-isolated but can be moved to final de-isolation. This
must be recorded on the ICC as part of the final de-isolation.
When a test run is required that involves the change of isolations applied under an ICC the
following procedure must be used:

EFFECTIVE CONTROL OF WORK
9.1
Cross referencing
Accurate cross-referencing between Permits and Supplementary Certificates is vital if a number
of tasks are to proceed simultaneously in safety.
9.1.1 Associated and affected permits
A number of tasks may proceed simultaneously in an area or on a system that spans a
number of areas. It is essential to establish that these tasks will not adversely affect one
another and that they can each be progressed safely at the same time.
Section 3 of the Permit to Work has a space allocated to Cross-references Certificates.
Any task that it is considered will be affected by the proximity or technique of other tasks
should be briefly described. This will give the AA the opportunity to consider adjacent
and associated tasks. At handover it will provide a reminder to the AA to discuss these
tasks with the PA. Additions to the list may be made by any of the Affected AA‟s who
sign in Section 5.
9.1.2 Connected Permits and Overlapping ICCS
When an item of plant/system has been isolated, a number of tasks may then proceed
on the basis of the ICC. A Confined space entry permit may also be raised after an ICC
has been granted for a confined space. It is essential that every Permit raised should be
cross-referenced to the ICC. Additionally, a number of ICC‟s may share common
isolations. These overlapping ICC‟s should be cross-referenced on each ICC.
The boxes in the control section of the ICC provide for these cross-references. Before an
ICC can be cancelled the AA signs a declaration that all connected Permits to Work and
Entry Permit have been CANCELLED and that all work is COMPLETE. This must be
checked thoroughly before the isolations under an ICC are lifted.
9.1.3 Update of Cross-references
It is vital that all cross-references are kept up to date. When a new Permit to Work is
being raised, consideration should be given by the AA to determine any area where
existing Permits may be affected by the proposed Permit. Should it be considered that
existing Permits will be affected they must be immediately recalled and the PA‟s briefed
as to the proposed task.
9.2
Permit Register
A permit register will be maintained in the permit office. All Permits & supplementary certificates
must be registered before they are valid. The Registry will consist of assigning a unique number
to the head of the Permit/Certificate, and entering the details of the document in the register.

The sections of the Registry Number consist of location or site identification & Permit or
Certificate number in series.
Each PTW & supplementary certificate will be entered separately in the register. Under the
individual permit entries the cross-references to the ICC under which the work is being
performed will be made. When a PTW & supplementary certificate, the PTWC must update the
Permit Register accordingly. He then signs the cancellation statement at the foot of each
document. The information in the registers must be kept up to date continuously and print out
must be made available for AA‟s.
During shift changeover the oncoming responsible person must visit the PTW office to receive a
full listing of work permits in use.
9.3
Permit Display
The PA copy of Permits must be displayed at the work site. If there are no means of displaying
the permit at the worksite, it should be kept on the worksite and accessible to all members of the
work party.
9.4
Site Plot Plans
Site Plot Plans shall be available in the permit office for easy visibility and control. These plans
should illustrate the various types of PTW activities ongoing and be colour coded to reflect the
PTW forms colours. This must be kept up to date constantly and be available for any personnel
to view. The plot plans are to be used continuously by the AA to assess adjacent activities and
also as a basis of discussions with affected AA‟s and PA‟s.
9.5
PTW planning meeting
To ensure effective work control is achieved it is vital that a robust structured permit to work
control & planning meeting is held every day to discuss the following days activities. All
responsible persons will attend the meeting and bring with them their prepared new permits. It is
also very important that site rules instruct PA‟s to submit the new permits to the AA in sufficient
time to allow the AA to carry out the required level of discussion, planning and site checks to
ensure that all the hazards and controls are identified.
The permit to work planning & control meeting should also have access to the permit listings or
permit location boards to view all other activities ongoing, each new permit submitted should be
reviewed to ensure there are no known conflicting activities, any concerns with conflicting
activities should be discussed at this meeting.
AA‟s who may be affected by the performance of the proposed work should be consulted. They
may provide additional precautions to be taken. The AAA‟s endorse the permit also to signify
their approval for the proposed task to proceed. The AA & site controller will check that all
AAA‟s have been identified and informed of the proposed work. If satisfied with the procedure
and precautions outlined in Section 2 and he signs to endorse the Permit.
The site controller gives permission for the work to proceed at the time, and under the
conditions specified on the permit, all of which are necessary in order to ensure the safety,
health and welfare of the personnel involved.
The permits are then sequentially numbered by the PTWC, and the permit number is cross
referenced with any identified supplementary certificates. The permit expiry date and time are
entered, being a maximum of 7 days.
9.6
Approval of personal isolations
Under certain circumstances, e.g., for short duration low risk tasks, the AA may authorise the
PA to carry out personal isolation / de isolations. The AA must clearly identify in section 3 the

permit that personal isolation area allowed. This must be discussed in the planning stage of the
permit along with the PA. For full details on personal isolations see AZSPU-HSSE-DOC-00048-
2, Energy Isolation Electrical and AZSPU-HSSE-DOC-00049-2, Energy Isolation Process.
The rules for approval of a personal isolation are as follows:
 The task and isolation are performed by the PA within one shift; the PA may receive
assistance from others but he shall be present at all times when work is in progress.
 Before approval, the AA must be satisfied with the competence of the PA to both carry
out the isolation and execute the work. Personal isolations shall only be carried out by
those deemed to be a competent Isolating Authority. Where isolation involves
competency in more than one discipline e.g. electrical, mechanical, instrument or
process, personal isolation is only permissible if the PA has all the appropriate isolating
competencies.
 The same isolation standards apply to personal isolations other than the completion of
the ICC.
 The isolation points shall be labeled with a personal isolation label and locked.
 If the worksite is left unattended, the plant shall be left safe, including capping or
plugging of any open ends.
 The intended points of isolation must be clearly stated on the Permit or supporting
documentation.
 If isolation is in place beyond one shift an ICC must be raised.
An ICC MUST be raised whenever these conditions are broken.
Competent electrical persons (CEP) may carry out personal electrical isolations without an ICC,
provided work is completed by that person in their own shift. Each CEP will be issued with one
unique personal electrical isolation lock and tag, which will be engraved with that person's name
and 'personal padlock'. To carry out electrical work without the need of an ICC, full electrical
isolation must be achieved by the application of one personal ID tag or padlock. An approved
isolation - Do Not Operate and Caution Notice shall be attached secured by the tag or lock.
Personal electrical isolation is not allowed on:
 High voltage (HV).
 Safety systems e.g. Fire and Gas or Emergency Shutdown (ESD) panels or sections
thereof.
 Equipment fed from more than one source.
9.7
Control of Over-rides, Inhibits & Alarm disables
It is essential for safety and efficiency purposes that any task requiring inhibition is clearly
communicated to all those involved in the task, this includes those that will be required to
perform the placement and removal of inhibits. Sites may vary how inhibits, over-rides and
alarm disables are controlled within the PTW process. Control of Over-rides, Inhibits and Alarm
Disables Procedure

inhibits should be recorded and assessed.
9.8
PTW Attachments and checklists
Attachment & checklists sheets can be utilised in the PTW system, when added to the existing
controls on the permit to work certificate the sheets will ensure that generic controls are not
overlooked. They will provide the PA & AA with additional information on the tasks and hazards
that may be encountered.

9.9
Site Alarms & Stopping Work
If work is interrupted in site / installation conditions, appropriate control measures must be re-
assessed before work is allowed to re-commence. Interruptions may include any break in work
when conditions may have significantly changed, alarms, emergency situations and shift
changes.
After any break in activity, the conditions and control measures must be reassessed as
compliant with the current permit by a competent person before any work can restart.
Where conditions or control measures are seen to have changed, work must not restart until the
situation has been assessed by a competent person and conditions returned to those required
by the permit. If this cannot be achieved, a new permit may be required.
Note: There may be some site specific exceptions to this, e.g. the operation of a diesel power
unit during wire-line operations where the wire-line crew are in radio / phone contact with the
CCR. In this case the well is to be made safe as per procedures and communication maintained
with CCR. These exceptions should comply with section 1.3 Deviations & Local Rules.
9.10 Suggestion for Permit System Change
To enable the Permit to Work System to evolve, as a result of ongoing experience and changes
to how work is controlled, a method of feeding back comments is provided. The suggestion form
(Appendix 15) should be completed and then passed to the custodian of this document.
Suggestion forms will be reviewed on an ongoing basis. Yearly, a report based on the collated
suggestions will be issued by HSE. These suggestions will be formally put up for
implementation at the next Permit System Review

1.
INTRODUCTION
1.1
PURPOSE
The main objective of this procedure is to provide a safe working environment
achieved by the erection of well designed, well erected work platforms as required at
all work locations.
A large number of accidents are caused by falls of persons and materials and
therefore a safe working place comes high on the list of priorities in accident
prevention. It is also an established fact that higher productivity is achieved by
persons who feel safe and are therefore able to devote their whole attention to the
job in hand.
Scaffolding, staging, access stairs, ladders and personnel hoists are to be erected,
inspected and recorded by suitably qualified and competent scaffolders only. At no
time should anyone who is not a competent scaffolder erect, dismantle any staging /
scaffolding unless he is working with and under the instruction of a competent
scaffolder.
1.2 SCOPE
The contents of this procedure are applicable to all BP owned and managed sites /
installations in Azerbaijan and Georgia. Contractors working on BP owned or
managed sites / installations are also responsible for alignment with this procedure.
This document does not replace the procedures prepared and adopted by specialist
contractors. Neither does it supersede any national regulatory requirements.
This procedure contributes to compliance with the “HSE expectations” contained in
“getting HSE right”, the „Golden Rules of Safety‟ and the Control of Work (CoW)
standard that the Hazards associated with BP activities are identified and that the
risks are assessed and managed.
All guidelines contained shall be regarded as the minimum requirements for BP
owned or managed sites / installations in Azerbaijan and Georgia.
The scope covers defined activities of BP and Contractors at all BP AzSPU sites and
installations.
1.3 LEGISLATION & STANDARDS
The aim of this Safe System of Work is to achieve ”no accidents”, “no harm to
people” and “no damage to the environment”. To achieve this aim, this SSOW
complies with National Legislation, the terms of the Production Sharing Agreement
(PSA) and mandatory BP Standards.
The best International Oil Industry practice has been adopted to reduce the level of
risk to ALARP.
In the absence of local regulations, BP Group Standards will apply. In addition,

appropriate UK and US regulations and industry best practice have been considered
in setting suitable goals and targets.
1.4 COMPANY REQUIREMENTS
It is a company requirement that all tasks are subjected to an assessment of risk to
demonstrate that risks have been reduced to as low a level as reasonably
practicable
(ALARP). This can be achieved by complying with the Company‟s
existing standards. Where compliance with Company standards cannot reasonably
be achieved, a formal level 2 Risk Assessment will be undertaken to identify any
additional controls and demonstrate that risks remain as low as reasonably
practicable. Whether by compliance with Company Standards or through level 2 Risk
Assessment, the Company‟s Golden Rules of Safety must be complied with. Golden
Rules are non-negotiable.
1.5 STOPPING UNSAFE WORK
To stop the continuation of potentially unsafe work at the earliest possible stage, the
Control of Work (CoW) Policy and this procedure for Scaffolding make it very clear
that all personnel are obliged and have the authority to “STOP” the work that they
consider to be unsafe.
1.6 DEVIATIONS
This procedure is written in sufficient detail that it should be able to be applied
consistently at all sites / installations. There may still be the requirement for some
local rules covering site / installation specific logistical/administrative arrangements
and local variations in responsibilities to reflect differences in organisational
arrangements. These local rules should not deviate from the core processes within
this document. Any form of deviation from this procedure, including but not limited to
local rules, shall be requested and authorised in accordance with SSOW, Deviations
from Regulations and Procedures (Doc. No: AZSPU-HSSE-DOC-00011-2).
1.7 DOCUMENT REVIEW
This document will be reviewed on an annual basis when users from the sites /
installations will have an opportunity to propose changes to the existing processes
and procedures. The document Technical Authority will be responsible for
coordinating this review.

requirements of risk assessment, permit to work conditions, and any risks or
hazards associated with the work activity
 That regular inspection is performed on all scaffolding activities to confirm that
conditions are suitable and sufficient and, that all personnel are in compliance
with this procedure, and that good housekeeping practices are being
implemented to limit or eliminate the potential for dropped objects.
 That the Performing Authority performs Risk Assessments, and conducts
Toolbox Talks associated with scaffold operations.
 That personnel involved is adequately trained to perform task

6 SCAFFOLDING DESIGN REQUIREMENTS

6.1.2 Wooden Boards
Wooden boards should be inspected to see that they are:
 Graded for scaffold use
Note: Wooden boards used for scaffolding must be specifically graded for
scaffold use by a nationally recognised grading agency.
 Is sound and in good condition
 Straight grained, free from saw cuts, splits, holes and any sign of
deterioration.
 In the case of 38mm thick boards, they are banded at either end or nail
plates fitted
6.2 CONSTRUCTION
6.2.1 National Requirements
The scaffold assembly must be designed to comply with local or United Kingdom
requirements, whichever is the higher.
6.2.2 Load Calculations

Frame spacing and mud sill size can only be determined after the total loads to be
imposed on the scaffold and the strength of the supporting soil or structure are
calculated and considered. A qualified person prior to the scaffold structure being
built must do this analysis.
Note: Manufacturers load carrying information shall be used for design
calculations
6.2.3 Construction, Dismantling and Alteration
Competent workmen under competent supervision must carry out the erection,
dismantling and alteration of scaffolding.
Plumb and level scaffold until connections can be made with ease. Do not force
members to fit. Be sure scaffold stays level and plumb as erection progresses.
During dismantling of scaffolding, tubes and fittings shall not be dropped to the
ground but always carefully lowered. Tubes shall be stacked flat and fittings collected
into bags or containers.
6.2.4 Support and Bracing
Scaffolding must be securely supported or suspended, and where necessary braced
to ensure stability. Unless constructed as freestanding independent scaffolding, it
must be rigidly connected with the building or structure (not to pipe work).
Ties, guys, bracing and/or outriggers may be needed to assure a safe stable scaffold
assembly. Determine the need for stability bracing.
If in doubt - ask.
The requirement for stability bracing is dependent upon:
 The height of the scaffold in relation to the minimum base width
Note: Freestanding Scaffolds with a height in excess of 4 times internally or 3.5
times externally, the minimum base width dimension must always be
secured to a rigid structure or seek alternative means of support.
Internally = no wind loading
Externally = relatively sheltered locations / minor wind loading
 Wind loads
 The use of brackets or cantilevered platforms
 Imposed scaffold loads.
Ties:
 The bottom tie must be placed no higher than four (4) times the minimum
base width and every four (4) meters vertically thereafter. Ties should be
placed as close to the top of the scaffold as possible and, in no case, more
than three (3) times the minimum base width of the scaffold from the top.
 Vertical ties should be placed at the ends of scaffold runs and at no more

than 4 meters horizontal intervals in between.
 Ties should be installed as the erection progresses and not removed until the
scaffold is dismantled to that height.
 Ties should be constructed to 6.25kn (slipload), unsheeted & 12.5kn sheeted
Guys:
 Each leg of a freestanding tower must be guyed at the intervals outlined
above or otherwise restrained to prevent tipping or overturning.
Note: Circular scaffolds erected completely around or within a structure may
be restrained from tipping by the use of "stand off" bracing members.
6.2.5
Planks change to boards
Work platforms must be fully boarded either with scaffold graded solid sawn
or laminated boards, in good sound condition, or with fabricated platforms in
good condition.
Each board must overlap the support by a minimum of 1.5 x thickness or be
cleated / securely tied for example, 3 metre board on 2.8 meter spans must
be cleated/securely tied.
Boards must not extend beyond the support by more than 4.5 x thickness.
Such overhangs should be separated from the work platform by guard-railing
so that they cannot be walked on.
Spans of full thickness, 50mm by 250mm scaffold grade boards, should
never exceed 2.6m. Loads on boards should be evenly distributed and not
exceed the allowable loads for the type of board being used.
Spans of normal thickness (38mm) boards should never exceed 1.5m.
Boards and/or platforms should always be secured to scaffolding to prevent
uplift or displacement due to high winds or other job conditions.
6.2.6
Guardrails and Toe Boards
The height of the top guardrails shall be between 910mm & 1150mm and
distance between the posts of guardrails should not exceed 2.7 metres.
Guardrails must be used on all open sides and ends of scaffold platforms.
Both top and mid-rails are required.
6.2.7
Transoms
A transom is a length of scaffold tubular used as a cross-member to support boards
on working lifts & provide lateral structural support on non-working lifts. Particular
care and attention shall be paid to the use of transoms. In particular:
 Transoms should overhang the support points by at least 150mm
 Transoms hangers shall be used with bolts fastened to support transoms on
frames

 Transom spans greater than 2.7m (very light duty), 2.4m (light duty) or 2.1m
(general purpose) require knee bracing and lateral support
 Transoms used as side or end brackets need special bracing.
Note: Transoms must not be used for the storage of materials.
6.2.8 Scaffold Access
Access must be provided to all work platforms. If it is not available from the structure,
access ladders, frames with built-in ladders, or stairways must be provided. When
frames with built-in ladders are used, cleated boards or fabricated boards must be
used at platform levels to minimize or eliminate platform overhang. Access ladders
must extend at least 1 metre above platforms.
6.2.9 Bridging Scaffold
Bridging between towers should not be done with boards or stages unless the
overturning moments have been compensated for & suitable guardrails, ties etc are
fitted.
6.2.10 Mud Sills and Screw Jacks
Mud sills must be of adequate size to distribute the loads on the scaffolding to the soil
or supporting structure. Sills should be level and in full contact with the supporting
surface.
Base plates or screw jacks with base plates must be in firm contact with both the sills
and the legs of the scaffolding. Compensate for uneven ground with screw jacks with
base plates. DO NOT USE unstable objects such as blocks, loose bricks, etc.
6.2.11 Dismantling Scaffold
Dismantling system scaffolding is more or less just the reverse of erection.
Dismantling guardrails is to be started at the top. Each piece shall be removed and
lowered as one unsecures it. Unsecured pieces must not be left in place, even for a
moment.
6.3 ROLLING SCAFFOLDS
The tower height must not exceed 3.5 times internally or 3 times externally the
minimum base dimension. Outrigger frames or outrigger units on both sides of the
tower may be used to increase base width dimension when necessary.
All casters must be secured to frame legs or screw jacks with a nut and bolt or other
secure means. Total weight of tower + men, materials & tools etc should not exceed
the capacity of the casters.
Screw jacks must not be extended more than 300mm above caster base. Tower
must be kept level and plumb at all times.
Horizontal/diagonal (plan) bracing must be used at the bottom and top of tower and
at least every alternate lift. Fabricated boards with hooks may replace the top

diagonal brace.
 All frames must be fully cross-braced
 Only prefabricated boards or cleated / tied boards should be used
 Casters must be locked at all times the scaffold is not being moved
7 INSPECTIONS AND SCAFFTAGS
7.1 INSPECTION
Inspection of the scaffolding is to be done before each use. In particular, check:
 The Scafftag - Pay particular attention to the load rating
 That the scaffold is assembled correctly:
o that it is level and plumb
o base plates are in firm contact with sills
o bracing is in place and connected
o platforms are fully boarded with guardrails in place
o safe access is provided
o that the scaffold is properly tied and/or guyed
7.2 SITE INSPECTION (PRE-CONSTRUCTION)
The job site should be inspected to determine ground conditions or strength of
supporting structure, and for proximity of electric power lines, overhead obstructions,
wind conditions, the need for overhead protection or weather protection coverings.
Special care is needed when scaffolding is to be erected on fill or other soft ground or
on frozen ground. These conditions must be evaluated and suitably provided for. Any
underground services and recorded subsidence are to be identified.
7.3 SITE INSPECTION (POST-CONSTRUCTION)
A competent person shall inspect scaffolding:
 Before it is first used and then at least once every week
 Following any alteration
 If it has been exposed to weather conditions likely to affect its strength or
stability.
Details of inspections must be recorded & must be auditable.

7.4 SCAFFTAGS
“Scafftags” shall be used on all scaffold structures, if complete or partially
complete/dismantled to indicate whether or not the scaffolding is safe to use.
Scafftags must be positioned prominently at access points to scaffold structures.
When scaffolding is incomplete (whether partly erected or dismantled) or considered
to be unsafe for any reason, the green scafftag must be pulled to display the red (Do
Not Use) tag.
In addition, access to the scaffold shall be barriered off as soon as practicable.
8 SCAFFOLDING USE
8.1 ACCESS
Use only the safe means of access provided. Do not climb bracing or frames not
specifically designed for climbing. If access is not provided, insist that it be provided.
8.2 WORKING SAFELY
8.2.1 Personnel Movement on Scaffold
While working on scaffold, personnel shall take care to climb safely. The following
work practices shall apply:
 When on ladders:
o face the rungs as you climb up or down.
o use both hands on the ladder and maintain „three point‟ contact (keep one
hand firmly on frame or ladder at all times)
o do not try to carry materials while you climb
o ensure footing and balance before releasing hand grips
o do not work on / from the access ladder
 Do not overload platforms with materials
Never add sheeting to a scaffold structure without consulting a qualified
person.
 Do not extend working heights by using boards guardrails or by use of boxes
or ladders on scaffold platforms
 Do not remove any component of a completed scaffold assembly. Alterations
shall only be carried out by suitably qualified / authorised personnel under the
supervision of a qualified person.
Note: Any removed or damaged component should be immediately replaced.

8.2.2 Dropped Objects
Only the required tools and materials for the job should be on scaffolding, any loose
cuttings or materials generated by the work should be removed immediately to
prevent any trips or slips. When the work scope is completed, the scaffold area
should be cleared of all tools and debris created from the work as the normal
housekeeping exercise at job completion.
Loose articles and materials must be kept to an absolute minimum on scaffolding
platforms. All necessary precautions must be taken to prevent objects from falling
from scaffolds, e.g. by use of toe boards, barriers, etc.
8.2.3 Cantilevered Platforms
Materials should never be placed on cantilevered platforms unless the assembly has
been designed to support material loads.
(These types of platforms cause
overturning and uplift forces, which must be compensated for. All frames should be
fastened together to prevent uplift an overturning moment compensated for with
counterweights or adequate ties).
8.2.4 Hoisting and Lifting
Scaffolds should not be used as material hoist towers or for mounting derricks unless
the assembly is designed for that purpose.
8.3 ROLLING SCAFFOLDING
 Do not ride manually propelled rolling scaffold. No personnel should be on
the tower while it is being moved.
 Lock all casters before getting on the tower.
 Work only within the platform area: do not try to extend overhead work area
by reaching out over guard railing.
 Do not bridge between two rolling towers with boards or stages.
 Secure all materials before moving scaffolds.
 Be sure floor surface is clear of obstructions or holes before moving
scaffold.
 Be sure there are no overhead obstructions or electric power lines in the
path of rolling scaffold.
 Rolling towers must only be used on level surfaces.
 Move rolling towers by pushing at the base level only. Do not pull from the
top.
8.4
“QUICK ERECT ALUMINIUM SCAFFOLDING
The use of “quick erect” aluminium scaffold systems can be used subject to local
controls such as restricting use to non-hazardous areas and appropriate storage.

Site specific procedure needs to be in place approved by Site Manager.
Due to a risk of an incentive spark should there be the presence of residual
aluminium on oxidised steel surface the use of equipment constructed using
aluminium alloys should be minimised and the limited use of such equipment is to be
permitted providing the following conditions:
 The use of aluminium based access systems should be subject to a specific
risk assessment
 Personnel using aluminium equipment should be aware of the hazards
associated with aluminium equipment and should also:
-
Avoid any unnecessary contact between aluminium alloys and rusty
steel (rubbing, dragging, hammering, bumping, dropping)
-
Portable equipment represents the higher risk, because of the
increased risk of dropping or banging such objects. The hazard of
aluminium on rusty steel is far higher than the hazard of rusty steel on
aluminium
-
Consider the movement of any portable equipment constructed with
light alloys to be a risk
-
Where possible and practicable, protect exposed areas of aluminium
on portable equipment
(edges or corners) in some appropriate
manner
 In non-hazardous zones where the chance of flammable atmosphere is
small, the introduction of light alloys could be called a negligible risk, except in
situations where heavy impact could be likely, or release of flammable
material is more likely.
9 LADDERS
Ladders may be used as a short-term alternative to scaffolding or as means of
access to scaffolding, providing the points listed in this section are considered.
Note: Ladders are potentially dangerous. The most common type of accident
occurs through a ladder slipping.
9.1 TRANSPORTING AND MOVING LADDERS
Unless space restrictions dictate otherwise, always carry ladders parallel to the
ground.
Do not move, shift, or extend a ladder while the ladder is occupied.
Note: Do not walk under a ladder
Tie ladders down securely when transporting.
9.2 MAINTENANCE
Ladders shall be inspected and maintained by a competent person. The record of

inspections shall be registered and not be longer than six months.
Keep ladders free of oil, grease, and other hazards.
Do not use any ladder with structural defects; properly tag the ladder with a "Do Not
Use" notice, withdraw the ladder from service and quarantine it.
9.3 PREPARATION
Inspect ladder prior to EVERY use.
Before using a ladder, ensure that it is the correct item of equipment for the job in
hand and that it is in good condition
Warning: Aluminium ladders must not be used in hazardous areas due to the
danger of sparks when the ladder impacts steel).
Use ladders only for the purpose for which they were designed
(refer to
manufacturer's labelling and recommendations).
9.4
SAFE USE OF LADDERS
Always place a ladder on a firm base, set the angle to 68 (1:4), or 75%.
Make sure the ladder projects well above the level at which the user
stands.
Ensure sufficient overlap between stages of extension ladders.
Secure the ladder in place wherever reasonably practicable.
A standby person should remain at the base of the ladder whenever the
ladder is in use.
Barricade traffic areas in vicinity of ladder use, and lock, barricade, or
guard doorways in which a ladder is placed.
Keep area around the top and bottom of ladder clear.
Use only non-conductive side rails around live electrical equipment.
Do not use top or top step for standing/stepping.
Do not stand on cross bracing.
Always face the ladder when ascending or descending.
Always maintain 3 points of contact with the ladder (2 feet/1 hand or 2
hands/1 foot should be in contact with ladder at all times).
Carry tools in zipped or closed pouches around waist; use a rope to raise
or lower large items such as tool boxes or materials.
Do not overextend sideways. Use the belt buckle rule: keep your belt
buckle positioned between the side rails at all times, which will maintain
your centre of gravity.
Never allow more than one person on the ladder at a time.
